We have some exciting news!
You are able to fill your forms online before your dental visit. This can be now done in the comfort of your own home. No more forgetting the name of the new medication you have been prescribed to put on your medical history, or trying to fill in all the paperwork for the whole family in a rush after being stuck in a traffic jam on the way to the practice!
An email and/or SMS will be sent to you a week before your appointment with an invitation for you to fill in your forms using the portal. All that is needed is your last name and your date of birth. All information will automatically be updated in your records at the Practice. Don’t worry all information is secure. The first time you fill in your forms you may need to fill in a new document (although if you have previously filled in your forms on our tablet in the Practice this will already be done). At each subsequent invitation to fill them in you will just need to review them. The portal can be used on your computer, tablet or phone. If we currently only have your email address and you would prefer to be able to fill it on your phone please let us have your mobile number.
You do not need to do anything to prepare to use this service, a link will be sent to you along with your normal reminder message.
We hope that this improvement to our service will be helpful to you. If you have any issues we are as always happy to help and we can still provide the forms to be filled in at the Practice when needed.
